## Job Description We are seeking a board-certified Physiatrist (PM&R) to serve as Medical Director, providing clinical oversight and leadership across a post-acute and long-term care network in the Central Illinois area. This position involves a flexible, remote-first structure with meaningful opportunities for hands-on clinical involvement, including rounding at an Acute Rehabilitation Unit (ARU) with approximately 30 short-term and 40 long-term beds. You will enjoy a high degree of autonomy, collaboration with multidisciplinary clinical and operational leaders, and a platform to shape care quality at scale. This role blends remote medical direction with periodic on-site support for complex rehabilitation procedures, making it ideal for an experienced physiatrist who values both flexibility and hands-on clinical engagement. In this role, you will guide physiatric care standards, ensure documentation quality and compliance, and collaborate closely with facility teams to elevate rehabilitation outcomes. You will serve as the clinical authority for PM&R across assigned regions. Our company specializes in providing dedicated physical medicine and rehabilitation services within skilled nursing facilities, a critical and underserved niche in post-acute care. ## Key Responsibilities - Provide clinical leadership as the physician-of-record for PM&R services across assigned facilities - Conduct structured remote chart reviews, documentation oversight, and compliance audits - Review and co-sign clinical encounters per state and payer requirements - Establish and maintain physiatric care standards, documentation expectations, and clinical protocols - Offer remote guidance and feedback to therapy teams, APPs, and facility leadership - Identify trends in documentation, care quality, and training needs; lead targeted improvement initiatives - Participate in regular virtual check-ins with clinical operations, compliance, and quality teams - Support onboarding of new clinicians and facilities - Stay current with PM&R guidelines, PDPM requirements, and post-acute regulatory updates - Provide occasional on-site support for complex procedures, advanced assessments, or facility needs ## Qualifications - Medical degree (MD or DO) from an accredited school - Board certification in Physical Medicine and Rehabilitation (PM&R) required - Active state medical license(s) in good standing; multi-state licensure strongly preferred - Substantial clinical experience in PM&R with post-acute/SNF/rehabilitation familiarity - Demonstrated leadership in medical director, department chair, or equivalent role - Strong working knowledge of PDPM, rehabilitation coding, documentation compliance, post-acute regulatory requirements - Excellent written and verbal communication suited to remote clinical leadership - High professionalism, sound clinical judgment, ability to set standards across a distributed network - EHR and clinical documentation systems proficiency ## Compensation - 1099 Independent Contractor - 70% of collections + collaborative stipend - High income potential (top earners $600K–$700K annually based on rounding volume) - Flexible scheduling and autonomy ## Schedule Flexible based on rounding preferences (2-4 days). Mix of remote oversight and on-site rounding. ## Location Central Illinois (Peoria / Bloomington / Springfield). Requirements
- MD or DO from an accredited medical school
