Urology has one of the smallest residency pipelines per capita of any surgical specialty, and demand has been outpacing supply for years. My typical urology search is hospital-employed for a community urology service line, or partnership-track in an established single-specialty group. Female pelvic medicine and reconstructive surgery (FPMRS), urologic oncology, and pediatric urology subspecialty searches have separate lead times.

Hospital-employed urology offers I close run $475K-$575K base with surgical and clinic productivity overlay. Single-specialty partnership economics regularly clear $700K-$875K by year three with ASC distributions and ancillary revenue from imaging and pathology. Subspecialty urology (urologic oncology, FPMRS, endourology) adds $50K-$125K to base in most markets. Robotic surgical volume is a meaningful comp lever in any urology contract.
Urology contract clauses I push back on
Urology contracts need scrutiny on robotic-case allocation (which can affect both productivity and surgical learning trajectory), ASC partnership-track timing, and call coverage for stone-emergency and oncology consults. Restrictive covenants in urology have been drafted aggressively in some PE-backed platform deals.
What has shifted in Urology hiring recently
Single-port robotic surgery, focal-therapy options for prostate cancer, and outpatient prostate procedures (HoLEP, Aquablation, Rezum, UroLift) have all reshaped urology workflow and economics. PE platform consolidation continues, with several urology platforms now in second-investment hold. And men's-health platforms have created a new direct-to-consumer adjacent market that some urologists pursue independently.
